If you have 2 ore more monitors, you might want to turn one on for the configuration. Sometimes quake3 doesn't like a secondary monitor to detect the resolution. I had this problem, and the only way to fix it was to do the following:
Go to the desktop properties - display. Go to the settings tab. Go to the resolution tab and turn off the rest of the options. Now go to the monitor tab. Change the resolution of your secondary monitor and leave the other one turned off. Now go back to the screen settings tab, apply the changes and you are done!
